Men's Team entry fee is $175
Women's Team entry fee is $125

Tournament prizes are as followed:
Men's (based on 24 teams) 1st place $500; 2nd place $400; 3rd place $300
Women's (based on 12 teams) 1st place $250; 2nd place $200; 3rd place $150.

Managers: I agree to pay the tournament entry fee of $175 for Men's team or $125 for Women's team and will mail the entry form to Juan Martinez at 4505 Appletree Circle Racine WI 53405 postmarked by Monday, August 22, 2011 to guarantee slot. NO EXCEPTIONS! Entries are on a first come, first serve basis. In addition, your FINAL ROSTER must be submitted by date of drawing. Drawing to be held on Thursday, August 25, 2011 at 6 PM at Sports Plex, 10116 Stellar Ave. Sturtevant, WI 53177. We reserve the right to change the format based on number of teams entering the tournament. We will guarantee each participating team 2 scheduled games. I also agree that my team will follow all the rules set forth by the Tournment committee. I am also responsible for informing my team members of these rules. I am aware that these rules stipulate that unsportsmanshiplike conduct will NOT be tolerated and my team, along with our fans will extend common courtesy to everyone involved in this event. I also realize that if tournament officials eject any of our players from the game, they will not only be ejected from the tournament, but they must also leave the premises.
